Reporting Tracking Requirements

NOTE: If ever you are not sure, complete the Incident/Accident Report Form and the Safe Schools Incident Reporting Form.

 

Situation

Complete for incidents when student is on a safety plan or when, in your professional judgement, feel that a plan may be required.

Responsibility

Teacher to record in safety plan.

Principal to review log on an ongoing basis.

Form/Policies


Situation

Implemented in situations where there is an ongoing risk of violence or safety concerns. 

Appendix D – Crisis Response plan for non-physical interventions (eg. calming techniques, evacuations, etc.)

Appendix S – Crisis Response plan for Physical Intervention (eg. blocks, containments, etc.)

Note: When safety plan is implemented, staff are not required to complete an incident/accident report for each incident, unless:

  • Staff are injured or there is a physical assault of staff.
  • There has been an increase in baseline behaviours (behaviours that have not been seen)
  • There has been a change in students’ triggers.

Responsibility

Principal responsible for development – should consult staff in development and must communicate to those who work directly with student.

Behaviour Log is used and review in the development of the plan.

Form/Policies


Situation

Staff involved in health and safety accident (eg. slip, trip, or fall)

Student verbal or physical Aggression directed at staff.

Major student verbal or physical aggression which posed a risk to staff. (Use your professional judgement)

Responsibility

All WRDSB staff responsible for reporting unsafe working conditions or incident/accident. (Part A)

Principal responsible for investigating and responding with staff cooperation (Part B)

Health, Safety, Security Department will follow up and record information into system database.

Form/Policies


Situation

Mandated under Education Act (Safe Schools Policy)

All WRDSB employees must complete and submit to administration for any incident where suspension or expulsion must be considered.

Responsibility

Staff must complete and submit to administration “as soon as reasonably possible”

Principal mandated to investigate and communicate results (communication is shared responsibility).

Form maintained in student OSR.

Situation

Complete only if containment was used with student and there was no Safety Plan implemented (or an Appendix S was not included in the plan).

Responsibility

Completed by person involved in containment.

Situation

Complete when you have sought medical medical attention as a result of an incident which occurred through the course of your employment.

Responsibility

Employee Responsibility to complete.

Form/Policies

 


Situation

Complete when a student has been injured.

Must be submitted online. Use this form to collect the data (if needed)

Responsibility

Employee responsibility (staff who witness or dealt with the situation).
